PROJECT SUMMARY 2003-IJ-CX-1032
The proposed study is national level exploratory descriptive study of transitional health care programs, including both physical and mental health care programs. This study aims to desribe the development, implemention and evaluation strategies of transitional health care programcurrently employed in State correction systems for soon to be released offenders wit HIV/AIDS, substance abuse, tuberculosis , hepatitis B and c, and mental illness. The study will also identify intergovernmental agency relationships and compare and contrast service goals and provider perceptions on transitional health care needs. Analysis will include description of programs and operational cost for running them. Project products will recommendations to other State coorrectional institutions for the development and implemention of transitional health care programs.
